Because you need to modify the existing overall_header.tpl (plain HTML syntax for the most part, it's fairly well commented so long as you're using a phpBB.com supplied template) in templates/subSilver (that's the defautlt, substitute your template name) rather than trying to send out a whole 'nother header, since the html standard only allows for one header per page
That document also contains the CSS for the template, so watch out for carelessly editing it.

It's the overall_header.tpl that you have to edit, just add your html or php code in that file (templates > Subsilver if you use that template), no need to add code in the page_header.php or index.php file.

Lame, yeah, thats the way I usually do it, just this time I wanted to lay a header that is common to the whole website to it, so I was hoping I could just include the php file. Looks like I'll have to do it another way then!!
